Job summary

Trilogy Health Services in Boonville, IN, is seeking a Memory Care Leader (CNA) to oversee the Legacy Lane dementia care program on our health campus. You will implement dementia care practices, lead staff education, and coordinate service plans for residents with cognitive impairment.

As the Dementia Champion, you will ensure safe, high-quality care, staff supervision, and collaboration with the Executive Director and families.

Qualifications

• High School diploma or GED/HSE required or preferred.
• Must possess and maintain a current, valid state CNA certification.
• 1–3 years of relevant experience preferred.

Responsibilities

• Collaborate with the Executive Director and leadership team to oversee daily operations of Legacy Lane programs.
• Lead initiatives to educate campus staff and the community on dementia care topics as Dementia Champion.
• Support efforts to maintain resident population according to memory care move-in/move-out criteria.
• Develop personalized Service Plans for memory care residents and guide families through the care continuum.
