Job summary

All Seniors Foundation is seeking a Licensed Clinical Social Worker (LCSW) to deliver psychosocial assessments, individual therapy, and clinical case management to seniors in their homes across Los Angeles County.

You will join an interdisciplinary team addressing depression, anxiety, family conflict, and end-of-life concerns while navigating CA LCSW regulations and Medicare standards. The role offers competitive salary and comprehensive benefits.

Qualifications

• Current CA LCSW license (or eligible within 90 days)
• MSW from accredited program
• 2+ years post-licensure clinical experience; geriatric population preferred
• Comfort with home-based and telehealth visit models
• Strong clinical documentation skills

Responsibilities

• Conduct comprehensive psychosocial assessments for seniors in home-based programs
• Provide individual psychotherapy (CBT, supportive, problem-solving) to seniors and families
• Lead family meetings around goals of care, advance directives, and caregiver stress
• Coordinate with nurses, physicians, and case managers on integrated care plans
• Connect seniors to community resources (housing, food, legal aid, mental health)
• Document clinical encounters in EHR per CA LCSW regulations and Medicare standards
