Job description

The Memory Counseling Program within Gerontology & Geriatric Medicine is seeking a Licensed Clinical Social Worker (LCSW) to provide specialized behavioral health services, case management, care coordination, and caregiver support for individuals living with cognitive impairment and dementia.

This role combines clinical social work, psychotherapy, care coordination, and resource navigation to support patients and caregivers across the continuum of care. The Clinical Social Worker will work closely with an interdisciplinary team to conduct psychosocial assessments, develop treatment plans, facilitate family meetings, connect families to community resources, and provide counseling and support services.

Key Responsibilities

• Conduct psychosocial assessments and mental health evaluations.
• Provide individual psychotherapy and supportive counseling for dementia caregivers.
• Perform case management and care coordination activities, including GUIDE Program visits.
• Develop and implement psychosocial treatment plans.
• Facilitate family meetings and caregiver support groups.
• Connect patients and families with behavioral health and community resources.
• Collaborate with physicians, nurses, and other healthcare professionals to support patient care.
• Document services and interventions in the electronic medical record.
• Assist with program outcome tracking and quality improvement activities.

Required Education & Experience

Education: Master of Social Work from an institution accredited by the Council on Social Work Education.

Experience:

• Clinical social work experience required.
• Minimum 3 years' experience working in a mental health setting with clinical diagnosis and treatment responsibilities.
• Experience working with older adults, geriatric populations, and individuals living with cognitive impairment or dementia strongly preferred.

Licensure

Full clinical licensure in the applicable state through the Social Work Certification and Licensure Board.

• Proficiency in DSM-5 criteria.
• Strong counseling, communication, assessment, and care coordination skills.
• Knowledge of community resources, behavioral health systems, and case management practices.
• Ability to work independently and as part of an interdisciplinary team.
