The Memory Counseling Program within Gerontology & Geriatric Medicine at Wake Forest University Baptist Medical Center is seeking a Licensed Clinical Social Worker (LCSW) to provide behavioral health services, case management, and caregiver support for individuals with cognitive impairment.

The LCSW will collaborate with an interdisciplinary team to conduct psychosocial assessments, develop treatment plans, facilitate family meetings, connect families to community resources, and document

Qualifications

• Master of Social Work (CSWE accredited) required.
• Minimum 3 years' clinical social work experience in mental health.
• Experience with older adults or dementia preferred.

Responsibilities

• Conduct psychosocial assessments and mental health evaluations.
• Provide individual psychotherapy and supportive counseling for dementia caregivers.
• Perform case management and care coordination activities, including GUIDE Program visits.
• Develop and implement psychosocial treatment plans.
• Facilitate family meetings and caregiver support groups.
• Connect patients and families with behavioral health and community resources.
• Collaborate with physicians, nurses, and other healthcare professionals to support patient care.
• Document services and interventions in the electronic medical record.
• Assist with program outcome tracking and quality improvement activities.
