Solera Senior Living is seeking a Memory Care Caregiver (CNA) for Modena Green Oaks in Illinois. You will assist residents with daily living activities, promote dignity and independence, and follow established care plans under supervision.

Qualifications

• Annual flu vaccination required (medical or religious exemptions considered).

Responsibilities

• Assist residents with Activities of Daily Living (ADLs) including transferring, mobility, bathing, diaper changes, peri-care, toileting, oral hygiene, grooming, dressing and linen changes.
• Support residents during meals, activities, appointments, and walks, using walkers or wheelchairs as needed.
• Document care provided, complete shift summaries, and communicate with the team for continuity of care.
