Job summary

The Alden Network in Rockford, IL seeks a dedicated CNA to support and assist in a Memory Care Unit, providing holistic, person-centered care for residents with dementia. The CNA will aim to maintain the highest quality of life for each resident under the facility’s Memory Care program.

Responsibilities include ongoing education, detailed knowledge of residents’ backgrounds, effective communication, and contributing to a therapeutic activity program.

Responsibilities

• Commit to continued growth as a knowledgeable and experienced healthcare provider in dementia care.
• Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other dementia care trainings.
• Become knowledgeable of each resident’s background and needs in detail.
• Utilize communication devices for residents to enhance communication.
• Provide a therapeutic, person-centered memory care activity program and adapt to residents' needs.
• Encourage family involvement through volunteering and visitation.
• Provide a clean, safe, non-hazardous unit.
• Assist in symptom management tracking and non-pharmacological interventions.
• Complete all required documentation.
• Maintain all policies and regulations.
• Provide feedback to Memory Care Director.
• Be flexible to changes in residents’ needs.
• Provide emotional, caring support to residents and families.
• Perform other duties as assigned.
