Job summary

Oakmont of Silver Creek in San Jose, CA, is seeking a Memory Care Caregiver for a full-time PM shift (2 pm–10 pm). The role supports residents with daily living activities, provides compassionate care, and participates in a warm, team-oriented environment.

Qualifications include a High School Diploma or GED and a willingness to train. Experience is preferred but not required. Benefits include medical, dental, vision, life insurance, 401(k) and paid time off.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years of age.
• Willingness to train; experience preferred but not required.
• Ability to interact with residents, families, and staff.
• Criminal background check and health screenings, including TB tests.
• Vaccination or PPE may be required per regulatory guidelines.

Responsibilities

• Assist residents in Activities of Daily Living per care plan.
• Respond promptly to residents' needs while maintaining dignity and safety.
• Communicate changes in resident condition to a supervisor.
• Maintain a safe environment following safety standards.
• Encourage teamwork with coworkers and other departments.
