Job summary

Sycamore Heights Health and Rehabilitation is seeking a Memory Care Activities Assistant to plan and coordinate memory unit activities, encourage participation, and accompany residents on outings. The role supports behavioral care for residents with memory-related needs and requires collaboration with the Memory Care Director and staff.

The ideal candidate will have strong organization, creativity, and a positive, professional demeanor.

Qualifications

• High school diploma or GED required or equivalent related work experience.
• Experience in activities preferred
• Dementia Certification preferred
• Ability to assume a leadership role in the activities department in the absence of the Memory Director
• Skill at working with individuals who have cognitive, physical, or sensory disabilities
• Moderate physical activity: up to 25 pounds, standing/walking, bending/stooping, computer work, travel to sites

Responsibilities

• Assist in planning, coordinating, and evaluating memory unit resident activities.
• Encourage resident participation in programs and activities.
• Escort residents on activities away from the property as assigned.
• Assist with registration of residents for trips and programs.
• Conduct activities under the Memory Care Director's direction.
• Maintain activity supplies and assist in purchasing as needed.
• Survey residents to determine interests and satisfaction.
• Maintain观察 and share information with staff.
• Follow budget guidelines and attend training and staff meetings.
