Job summary

Sycamore Heights Health and Rehabilitation is seeking a Memory Care Activities Assistant to plan, coordinate, and evaluate activities for memory care residents. You will encourage participation, escort residents on outings, and assist with trip registrations under the Memory Care Director's guidance.

Qualifications

• High school diploma or GED required or equivalent experience.
• Experience in activities preferred.
• Dementia Certification preferred.
• Ability to lead the activities department in the Memory Director's absence.
• Ability to work with individuals with cognitive, physical, or sensory disabilities.

Responsibilities

• Assist in planning, coordinating, and evaluating memory unit resident activities.
• Encourage resident participation in programs and activities.
• Escort residents on activities away from the property as assigned.
• Assist with registration of residents for trips and programs.
• Conduct activities as directed by Memory Care Director.
• Maintain activity supplies and assist with outside purchases when needed.
• Survey residents to determine interests and satisfaction.
• Observe residents and share information with staff.
• Attend required trainings, in-service, and staff meetings.
• Maintain a safe working environment and adhere to policies.
