Job summary

American Health Advantage in Indiana is seeking a Member Advocate to lead patient and family support across local service areas. You will coordinate with Insurance Producer after enrollment to ensure a seamless transition and act as the primary contact for plan questions and updates.

This role requires strong interpersonal skills, travel to nursing facilities within the region, and strict adherence to CMS regulations.

Qualifications

Experience in healthcare delivery with elderly patients.
Exceptional interpersonal and listening skills.
Ability to work independently and with a team.
Willingness to travel within the assigned region; valid driver’s license and proof of insurance.

Responsibilities

Coordinate with Insurance Producer after enrollment to ensure seamless transition and customer-service success.
Follow up with members after enrollment to answer questions and provide plan assistance.
Serve as front-line support with members, partner facilities and care team to meet needs.
Develop and maintain relationships with key network providers/facilities in the region.
Meet with the member and/or family at least monthly.
Follow up on items causing member dissatisfaction and report to plan.
Develop and maintain reporting on member status and facility calendars.
Ensure all interactions with members comply with CMS regulations.
Oversee services for a group of nursing facilities and ensure coverage for all members in those facilities.
Perform other responsibilities as assigned.
