Job Summary

The Medication Reconciliation Technician (MRT) will independently obtain medication and allergy histories for patients in the Emergency Department. This involves interviewing patients, families, and calling pharmacies and care facilities. The information will be entered into a computer system to assist healthcare providers in treatment planning and medication management, ensuring accurate medication lists and promoting patient safety.

The MRT reports to the Pharmacy Director or designee, who oversees program policies and continuing education. The technician performs clerical and clinical duties as assigned.

Minimum Education/License/Certification Requirements

• State Pharmacy Technician Certification through PTCB preferred but not required.
• At least one year of pharmacy technician experience preferred.
• Basic computer skills required.
• Experience with medication classification, math, and resources preferred.
• High School Diploma or equivalent required; Certified Pharmacy Technician preferred.
• Indiana State Pharmacy Technician license preferred.
• Basic Life Support (BLS) and CPI Certification required.
