The job entails assisting with the medication reconciliation process by interviewing patients and contacting pharmacies and physician offices to obtain an accurate medication history. In addition, the position entails being a resource for the pharmacist and assisting nursing and physicians with medication related requests.

Responsibilities

  1. Obtain and clarify patients' home/chronic medication list by contacting community pharmacies and extended care facilities, interviewing patient and family members to ensure proper identification of doses and drugs including prescription medications, over-the-counter drugs, and complimentary & alternative medications in an efficient and timely manner.
  2. Accurately document the home medication list in the electronic health record using the standardized documentation format. Communicate discrepancies identified via the medication history process with the pharmacist and make clarifications with the patient and prescribers as needed.
  3. Hand off effectively any medication history related issues and ensure proper follow-up and closure.
  4. Appropriately direct, triage, or communicate to the appropriate caregivers or staff all questions or inquiries from patient or families that are outside the scope of taking the medication history.
  5. Serve as a resource to the pharmacist regarding home medication histories and other follow-up as deemed appropriate by the pharmacist. Respond to nurses, physicians, or other requestors and appropriately manage, triage, or ensure follow-up in a timely manner regarding medication history information and/or medication history requests.
  6. Conduct consistent environment of care rounds to ensure safety and regulatory compliance. Identify opportunities for improvement related to safety, regulatory, or financial outcomes.
  7. Meet all Standard of Professional Behavior and other YNHHS Service Excellence Standards (including AIDET, 10-5, and no venting) and ensure high-quality customer service at each patient and team interaction. Demonstrate principles and conduct consistent with a High Reliability Organization.
  8. Meet all YNHHS attendance and punctuality standards.
  9. Participate in special projects and department goals as indicated.

Qualifications

Education

High School Diploma in combination with pharmacy experience OR High School Diploma and successful completion of an academic or military pharmacy technician training program. Certification by the national Pharmacy Technician Certification Board (PTCB), or equivalent, required on or within 6 months of hire.

Experience

6 months Pharmacy technician experience or healthcare experience required; retail/medication history experience preferred. Completion of an academic or military pharmacy technician training program will be considered equivalent to one year of pharmacy technician experience. Excellent customer service and communication skills are required.

Licensure

Current registration/licensure or eligibility for registration as a Pharmacy Technician in the State of employment, as outlined by the respective State regulatory agencies. Active Pharmacy Technician Certification Board (PTCB) certification, or equivalent, is required on or within 12 months of hire.

Special Skills

Must be registered or eligible for registration as a Pharmacy Technician in Connecticut, able to prioritize demands, work independently, show flexibility and teamwork. Must have excellent communication skills, proficiency in mathematical calculations, accuracy, problem solving, attention to detail, knowledge of computer applications, and ability to learn quickly in a fast-paced environment. Capable of basic troubleshooting with computer and technology equipment.

Physical Demand

May be required to work mandatory on-call shifts, including days and evenings. Considered essential personnel and expected to work during inclement weather.
