Job description

Position Objective:

Under the direct supervision of a Maryland-licensed pharmacist and in coordination with other technicians, the Pharmacy Technician-Medication Reconciliation (Med Rec) performs a comprehensive admission medication history for patients in predetermined focus areas of patient intake. An accurate medication list is created to enable effective medication reconciliation. This role requires knowledge of commercially manufactured drug products available on the retail market, meticulous attention to detail, excellent communication skills, and strong customer service abilities.

Essential Job Duties:

  1. Demonstrates in-depth knowledge of drug generic-trade names, dosage forms, and strengths marketed, with familiarity with herbals and OTC medications.
  2. Uses appropriate pharmaceutical-medical terms, abbreviations, and symbols common to pharmacy practice.
  3. Performs calculations required for dosage determinations and therapy duration.
  4. Maintains proper telephone etiquette during medication history validation calls and follows up appropriately.
  5. Interacts professionally with patients, families, pharmacy colleagues, and healthcare team members, adhering to the hospital dress code.
  6. Maintains patient confidentiality.
  7. Participates in orientation and training of new Med Rec staff, providing clear instructions and reviewing work for accuracy.
  8. Meets accuracy and volume standards, contributing to departmental metric goals, following research methodology for background information validation.
  9. Accepts instruction and coaching professionally, improving skills without constant supervision.
  10. Collaborates positively with ER staff and complies with PPE and restricted access policies.

Educational/Experience Requirements:

The minimum educational and experience requirements are:

  • High school diploma or equivalent.
  • Preferred: Pharmacy experience in hospital or retail settings.

Required License/Certifications:

  • Preferred: National Pharmacy Technician Board Certification (PTCB).
  • Current registration as a Pharmacy Technician or Intern with the Maryland Board of Pharmacy.

Working Conditions, Equipment, Physical Demands:

Employees may be exposed to blood-borne pathogens.

Physical Demands – Medium

Requires exerting up to 50 pounds occasionally, 30 pounds frequently, and 10 pounds constantly. Specific vision abilities include close, distance, color, peripheral, depth perception, and focus adjustment.
