Job description

Summary:

Under the supervision of a Pharmacist and the direction of the Medication History Pharmacy Technician Group Leader (RIH TMH only), the Pharmacy Medication History Technician plays a key role in the pharmacy-managed medication reconciliation practice model. The primary responsibility is to accurately obtain and compile current, detailed, and complete home medication history lists and allergies for patients admitted to the hospital. This involves using all relevant sources, including patient and family interviews, outpatient pharmacies, extended care facilities, and provider offices, to gather the most current information. The technician documents this information in the electronic medical record (EMR) and reviews patient allergy information, updating as needed. They interact effectively and compassionately with patients, families, and healthcare providers at all levels, contributing to continuous quality improvement activities.

Responsibilities:
  • Obtain and compile current, detailed, and complete home medication history lists, including drug name, dose, form, frequency, and route of administration for prescription, over-the-counter, and alternative medications.
  • Obtain and compile patient allergy information, including reactions experienced.
  • Use all relevant sources to gather the most current medication and allergy information, including interviews and contact with pharmacies and providers.
  • Communicate medication list discrepancies to providers or pharmacists, follow up on unresolved issues, and refer suspected medication errors to a pharmacist.
  • Serve as a resource for pharmacists regarding medication histories and related information.
  • Document verified medication lists and allergy information accurately in the EMR in a timely manner.
  • Respond appropriately to requests from healthcare providers and staff, managing triages and follow-ups.
  • Provide high-quality customer service during patient and family interactions.
  • Direct or communicate questions outside the technician's scope to the appropriate staff.
  • Participate in quality assurance, assessment, and safety initiatives related to medication reconciliation.
  • Rotate through all applicable work assignments and shifts.
  • Demonstrate professionalism and acknowledge responsibilities for self and others.
Experience & Basic Knowledge:
  • High school diploma or equivalent; college coursework preferred.
  • Maintain current Rhode Island Pharmacy Technician license.
  • Hold and maintain National Pharmacy Technician Certification (CPhT) or have at least one year of outpatient pharmacy experience; sit for the CPhT exam within 6 months of hire and obtain certification within 12 months.
  • Minimum one-year outpatient pharmacy technician experience.
  • Strong customer service, communication, and active listening skills.
  • Proficient in computer skills, including Microsoft Office and internet applications.
Work Environment & Physical Requirements:
  • Based initially in the Emergency or Admitting Department.
  • Ability to focus in a fast-paced environment, with extended standing and walking.
  • Visual and manual dexterity for computer use and data entry.
  • Successful passing of a drug screen in accordance with Rhode Island law.
Independent Action & Supervision:
  • Performs independently within department policies.
  • Refers complex issues to supervisor as needed.
