Northwell Health seeks a Medication History Technician to interview patients and families to obtain complete home medication histories. You will pull external history from patient pharmacies using available technology, ensuring accuracy and timeliness.

The role requires completion of PTCB certification within 1 year, valid Pharmacy Technician license, and CT/NY compliance within stated timelines. Prior retail pharmacy experience is a plus; strong computer and multi-tasking skills are essential.

Qualificações

Must possess high school diploma or equivalent.
PTCB certification within 1 year is required.
License as a Registered Pharmacy Technician is required.
Connecticut Dept. of Consumer Protection within 30 days for CT workers.
Board of Pharmacy for NY within 6 months for NY workers.
Retail pharmacy experience is a plus.
Computer skills and ability to multi-task essential.

Responsabilidades

Obtain and clarify patients' home medication lists (prescription, OTC, herbal).
Coordinate with nurses, physicians, pharmacists to manage med history information.
Record allergies and intolerances; document in EMR with symptoms and severity.
Direct questions to appropriate caregiver outside MHT scope.
Identify and participate in quality assurance to ensure accurate med history.
Fulfill all compliance requirements related to the position.
Perform other duties as assigned.
