Job summary

Northern Dutchess Hospital is seeking a Medication History Technician to work under the supervision of a pharmacist. You will obtain medication histories from patients and families, and pull external fill history from pharmacies.

Responsibilities include documenting home meds, allergies, and communicating questions to care teams. You will support proper EMR documentation and participate in quality assurance.

Qualifications

• Obtain and verify home medication lists, including prescriptions, OTC, and herbal products.
• Respond to requests to manage, triage and follow up on med history information.
• Record allergies and intolerances in the EMR with symptoms and severity.

Responsibilities

• Obtains and clarifies patient's home medication list, including prescription, OTC, and herbal products.
• Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up promptly regarding med history information.
• Obtains patient's allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR), including symptoms, reactions, and severity.
• Directs, triages, and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner outside the scope of the MHT.
• Identifies and participates in ongoing quality assurance and improvement processes to obtain an appropriate med history list.
• Fulfills all compliance responsibilities related to the position.
• Maintains and Models Nuvance Health Values.
• Demonstrates regular, reliable and predictable attendance.
• Performs other duties as required

Purpose:

Under the supervision of a pharmacist, the medication history technician (MHT) will obtain medication histories through interviews with the patient/family/care partner. The MHT will use technology to pull in external fill history from patients' pharmacies.

Essential Responsibilities:

1. Obtains and clarifies patient's home medication list, including prescription, OTC, and herbal products. Any bottles brought in will be promptly returned to the patient's family/care partner after all important information has been transcribed.
2. Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up promptly regarding med history information.
3. Obtains patient's allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR), including symptoms, reactions, and severity.

Required:

• High School Diploma or equivalent
• Minimum Experience: 1 year

Desired:

• Retail Pharmacy Technician experience.
• Computer skills, ability to multi-task and work independently.

License, Registration, or Certification Requirements:

• PTCB Certification within 6 months
• License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months
