Job summary

Northern Dutchess Hospital is seeking a Medication History Technician to obtain med histories from patients and coordinate with care teams. The role supports accurate EMR documentation and patient safety in the Pharmacy department.

Under supervision, the MHT will interview patients/families, collect home medication lists, and ensure compliance with privacy and quality standards. Requires NY Pharmacy Technician license within 6 months and PTCB certification within 6 months.

Qualifications

• High School Diploma or equivalent.
• Minimum Experience: 1 year.
• Retail Pharmacy Technician experience (Desired).
• Computer skills, ability to multi-task and work independently (Desired).
• PTCB Certification within 6 months (Required).
• License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months (Required).

Responsibilities

• Obtains and clarifies patient’s home medication list, including prescription, OTC, and herbal products.
• Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up regarding med history information.
• Obtains patient’s allergies & intolerances and communicates to appropriate caregiver for EMR documentation.
• Directs and communicates questions from patient/family outside MHT scope.
• Identifies and participates in quality assurance to obtain an appropriate med history list.
