Wellstar Health System is seeking a Medication History Specialist for the Emergency Department. You will obtain comprehensive home medication histories and document them in the electronic medical record, collaborating with clinicians to ensure accuracy and safe patient care.

Requires CPHT and PHT certifications, a minimum of 2 years pharmacy technician experience (retail preferred), and strong communication skills. Day shift, on-site at Wellstar facilities in Georgia.

Formación

• Minimum 2 years pharmacy technician experience, retail experience preferred.
• CPHT and PHT certifications required upon hire.
• High school diploma or GED; may require additional on-the-job training.

Responsabilidades

• Obtain and document the best possible medication history for admitted patients.
• Enter home medications into the electronic medical record and ensure accuracy.
• Coordinate with ED staff, physicians, and pharmacists to resolve discrepancies.
• Provide professional, compassionate patient interactions and maintain quality metrics.
