Job summary

Mercy in Des Moines, IA, is seeking a Pharmacy Technician to provide essential support by obtaining and verifying initial medication histories and allergy information from patients and families. You will document detailed medication lists and allergies into the EMR and work with a pharmacist on uncertainties.

The role requires completing competency sets, coordinating with care teams across ED, inpatient, and outpatient areas, and performing other duties as assigned in Iowa law.

Qualifications

• High school diploma or GED required when nationally certified through NHA.
• PCTB nationally certified technicians do NOT require HS diploma or GED.
• Proof of completion of Mandatory Reporter abuse training specific to population served within 3 months of hire.
• National Pharmacy Technician Certification; Minimum of 2000 hours of pharmacy technician experience; and Registered as a Certified Pharmacy Technician with the Iowa Board of Pharmacy at time of hire.
• Registered Pharmacist-Intern with the State of Iowa at the time of hire.

Responsibilities

• Successfully complete Medication History Pharmacy Technician Competency/Skill Set.
• Interview patients and/or their designated family/caregivers as soon as possible after presenting for care to Mercy clinical areas (Emergency Department, inpatient and outpatient).
• Obtain and record in the electronic medical record a current list of medications including prescriptions, OTCs, vitamins and supplements, and sources.
• Recording should include name/location of patient’s ambulatory pharmacy, medication name, dosage form, strength, dose, timing, and sources.
• Clarify uncertainties and compile as complete a medication history as possible using patient/family information, ambulatory records, clinic records, long-term-care records, and previous encounters.
• Obtain and record in EMR an allergy history, clarifying the nature of the allergic reaction.
• Consult with a pharmacist for unresolved uncertainties related to a patient’s medication and allergy history.
• Other duties as assigned by management.

Essential Key Job Responsibilities:
