Job summary

MercyOne seeks a Pharmacy Technician to support patient care areas by obtaining, verifying, and recording initial medication histories from patients or designated caregivers. The role includes interviewing patients, documenting current medications, allergies, and sources in the electronic medical record, and collaborating with a pharmacist to resolve any uncertainties.

Qualifications

• High school diploma or GED required when nationally certified through NHA.
• PCTB nationally certified technicians do NOT require HS diploma or GED.
• Proof of Mandatory Reporter abuse training within 3 months of hire.
• National Pharmacy Technician Certification; minimum of 2000 hours experience; Iowa Board of Pharmacy registration at hire.
• Registered Pharmacist-Intern with the State of Iowa at time of hire.

Responsibilities

• Complete Medication History competency/skill set.
• Interview patients and/or family/caregivers soon after presenting for care.
• Record in EMR current medications including prescriptions, OTC items, vitamins and supplements and sources.
• Record medication details: name, location, dosage form, strength, dose, time of most recent dose, sources.
• Clarify uncertainties and compile a complete medication history from multiple records.
• Record allergy history and clarify nature of reactions.
• Consult with a pharmacist for unresolved medication or allergy questions.
• Perform other duties as assigned by management.

Provide technical support in obtaining, verifying for accuracy, and recording initial medication histories from patients &/or their designated family/caregivers upon presentation to Mercy patient care areas. Includes conducting patient &/or family interviews focused on obtaining current medication regimens including prescription andover-the-counter(non-prescription) medications, as well as vitamin and herbal supplements.

Perform other functions of a pharmacy technician registered in the State of Iowa and within the scope of Iowa law as needed.

Essential Key Job Responsibilities:

• Successfully complete Medication History Pharmacy Technician Competency/Skill Set.

• Interview patients &/or their designated family/caregivers as soon as possible afterpresentingforcare to Mercy clinical areas (includesthe Emergency Department, as well as inpatient and outpatient areas).

• Obtain & record in the electronic medical record a current list of medications toinclude:prescription medications,over-the-counter(non-prescription) medications, vitamin and nutritional supplements, as well as the source(s) for this information.

• Recording should include the following: name/location ofpatient’sambulatory pharmacy, medication/supplement name, dosage form, strength, dose, day/time of most recent dose, and source(s) of the information being recorded.

• Attempt to clarify as many uncertainties and compile as complete a medication history aspossible utilizingpatient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facilityrecordsand information fromprevioushospital encounters.

• Obtain & record in the electronic medical record an allergy history for each patient. In doing so, clarify& recordthe nature of the allergic reaction when necessary.

• Consult with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication& allergyhistory.

• Other duties as assigned by management.

Minimum Qualifications:

• High schooldiploma or GEDrequiredwhen nationally certified through NHA

• PCTB nationally certified technicians do NOTrequireHS diploma or GED

• Proof of completion of Mandatory Reporter abuse training specific to population served within 3 months of hire.

• National Pharmacy TechnicianCertification; Minimum of2000hoursof pharmacy technician experiencerequired; andRegistered as a Certified Pharmacy Technician with the Iowa Board of Pharmacy at time of hire.
