Job summary

Gobravvo is seeking a dedicated Nurse Practitioner (NP) to join our value-based healthcare team in Dayton, Ohio. The role focuses on Medicare populations, preventive care, and improved patient outcomes within a collaborative clinic supported by scribes, medical assistants, and social workers.

The NP will provide comprehensive primary care, use Canopy and Greenway EMR systems, participate in physician partnerships, and access on-site labs and supporting patient transportation services to enhance

Qualifications

• Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• Active, non-probationary state NP license.
• National certification as Family NP, Adult-Gerontology or Gerontological NP.
• Active DEA license.
• Experience with Medicare populations is advantageous.

Responsibilities

• Provide comprehensive primary care to Medicare patients using a value-based care approach.
• Conduct regular patient visits, delivering high-quality care and preventive services.
• Complete health risk assessments and manage chronic and acute conditions.
• Collaborate with a care team including scribes, medical assistants, and social workers.
• Utilize EMR systems (Canopy and Greenway) for documentation.
• Engage in preventive health initiatives and patient wellness activities.
• Coordinate with specialty providers as needed.
• Pursue ongoing professional development and quality improvement initiatives.
