Job summary

The Institutes Of Applied Human Dyn in the Bronx, NY is seeking an Entry Level Direct Care Worker to support individuals with intellectual and developmental disabilities. Responsibilities include scheduling medical appointments, coordinating transportation, and assisting with daily care.

You will maintain records in the electronic medical record system, monitor medications and supplies, and report clinical findings to nurses.

Qualifications

• High School Diploma or GED required.
• Must have a current and clean New York State Driver’s License and ability to drive a 15 passenger van.
• AMAP, SCIP, CPR, and First Aid certifications (or ability to obtain within six months of hire).
• Knowledge of OPWDD policies and state/federal regulations.

Responsibilities

• Schedule medical appointments for individuals and arrange transportation.
• Ensure medications are ordered, documented on the medication administration form, and provided as directed by clinicians.
• Perform regular checks on medications and medical supplies to ensure there is a sufficient supply.
• File and maintain medical records in accordance with agency policies and document all activities in the EMR system.
• Provide direct care as needed, ensuring individuals’ needs are met professionally and compassionately.
• Escort individuals to and from medical appointments and prepare them for clinician visits.
• Report pertinent clinical findings to the nurse or clinician as needed to ensure proper care.
• Regularly check and replenish medical supplies to ensure residents' needs are met consistently.

What You’ll Do

Qualifications

• High School Diploma or GED required.
• Previous experience as a Direct Support Professional (DSP), Nursing Assistant, or Patient Care Assistant is a plus.
• Strong organizational skills, ability to multitask, and the ability to work under pressure.
