Job summary

Trinity Health At Home seeks a part-time social worker to conduct psychosocial assessments and develop individualized, patient-centered care plans in collaboration with physicians, patients, families, and the interdisciplinary team.

The role involves identifying barriers to care, providing counseling, and helping patients understand medical recommendations while collaborating with the care team to adjust plans as needs evolve.

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited program.
• Current MSW license as required by state regulations.
• Current CPR certification.
• Minimum of two years of social work experience in a healthcare setting.
• Ability to provide services to acutely and chronically ill patients.
• Strong communication, assessment, and critical thinking skills.
• Commitment to patient confidentiality and professional ethics.

What You Will Do

• Provide counseling and support to patients and families experiencing complex social or emotional distress.


• Assist patients and caregivers in understanding and following medical recommendations.


• Collaborate with physicians and healthcare professionals to address factors impacting patient health and well-being.


• Continually evaluate patient needs and modify care plans as appropriate.


• Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings.
