Trinity Health At Home – Grand Haven, MI is seeking a Hospice Medical Social Worker (MSW) to provide consultation and services to homebound patients, helping address complex social and emotional issues that affect medical regimen adherence.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• State of Michigan LCSW-eligible license required.
• Two years of social work experience in a health-care setting.
• Ability to assist acutely and chronically ill patients and complete written/verbal/observational tasks.

Responsibilities

• Provide social work consultation and service to homebound patients.
• Coordinate with care teams to address social and emotional barriers to treatment.
• Document assessments, plans, and progress notes for each patient.

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education.
• State of Michigan Full Masters Social Work License Required
• Two (2) years of social work experience in health care setting.
• Ability to provide service to acutely and chronically ill patients
• Ability to complete written, verbal, and observational tasks indigenous to the position.
