Job summary

Community Health Network (CHN) is seeking an experienced Medical Social Worker to provide compassionate support to terminally ill patients and families in their homes around Norton, MA. The role entails working Monday through Friday, 8:00 a.m.

to 4:30 p.m., with no on-call or weekend shifts, and mileage reimbursement as part of the compensation package. Earn great benefits including Health, Dental, Vision, 401(k), and Company paid Life Insurance.

Qualifications

• Current MA LICSW credential.
• Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
• Hospice experience preferred.

Responsibilities

• Plan and provide supportive services to terminally ill patients and families.
• See patients in their homes in the Norton/ Taunton/ Dedham/ Wrentham area.
• Collaborate with interdisciplinary team and report to Clinical Manager.

Well established hospice agency has a need for an experienced Medical Social Worker to see patients in their homes in and around the Norton/Taunton/Dedham/Wrentham, MA area.

• Full-time, Monday-Friday
• 8:00 a.m. - 4:30 p.m.
• No on-call or weekends required!

As a member of the interdisciplinary team, the Medical Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services to terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying.




Requirements:

• Current MA LICSW
• Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
  • Hospice experience preferred.
• Must be comfortable participating in a collaborative/team environment.
