Medical Social Worker, Home Health Visits

BAYADA Home Health Care is seeking an energetic and experienced Licensed Medical Social Worker (MSW) for a per diem position providing home health visits from our York Visits Office. This office services clients throughout York, Adams, and Fulton County. Prior home care experience is preferred but not required. As a Medical Social Worker, you will be a vital part of a multidisciplinary team delivering skilled nursing and rehabilitative care to enable clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Understanding social and emotional factors affecting clients’ health.
3. Providing clinical social evaluations and planning interventions.
4. Referring clients to community resources for practical and environmental needs.
5. Advocating for clients and families within community systems.
6. Participating in care planning with the healthcare team.
7. Developing and maintaining community resource contacts.
8. Educating patients and families about health, safety, and independence.
9. Documenting care activities accurately using electronic devices.

Qualifications:

1. Licensed by the PA Board of Social Work Examiners (LMSW or LISW).
2. Master’s degree in Social Work with at least 2 years of community experience.
3. Accredited by the Council on Social Work Education.
4. Home care experience is a plus.
5. Effective communication skills in English.
6. Ability to work independently and manage time.
7. Strong interpersonal skills.
8. Computer proficiency; EMR experience preferred.
9. Willingness to travel as needed.
