Job description

BAYADA Home Health Care

We are currently seeking an energetic and experienced Licensed Medical Social Worker, MSW for a per diem opportunity performing home health visits in our York Visits Office. This office services clients across York, Adams, and Fulton County.

Prior home care experience is preferred but not required. As a Medical Social Worker, you will be a vital part of a multidisciplinary healthcare team that provides skilled nursing and rehabilitative care, enabling clients to receive necessary medical care at home.

Responsibilities include:

1. Making home visits to clients in designated geographic areas.
2. Understanding social and emotional factors affecting our clients' health.
3. Providing clinical social evaluations and planning appropriate interventions.
4. Referring clients to community resources to meet their practical and environmental needs.
5. Advocating for clients and families to navigate community systems.
6. Participating in developing the overall care plan with the team.
7. Developing and maintaining contracts with community agencies.
8. Educating patients and families about community resources to promote health and safety.
9. Documenting observations and interventions accurately using electronic devices.

Qualifications:

1. Licensed by the PA Board of Social Work Examiners at the LMSW or LISW level.
2. Master's degree in Social Work with at least 2 years of community-based experience.
3. Accreditation by the Council on Social Work Education.
4. Home care experience is a plus.
5. Effective communication skills in English.
6. Ability to work independently and manage time efficiently.
7. Strong interpersonal skills.
8. Proficient computer skills; EMR experience preferred.
9. Ability to travel as needed.
