Job Opportunity: Medical Social Worker (MSW) - Part-Time or Per Diem

BAYADA Home Health Care is seeking an experienced Licensed Medical Social Worker (MSW) for a part-time or per diem role, performing home health visits in our Harrisburg Visits office. This office services clients across Dauphin, Cumberland, and Perry Counties.

Prior home care experience is preferred but not required. As a Medical Social Worker, you will be a vital part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated territories.
2. Understanding social and emotional factors affecting clients' health.
3. Providing clinical social evaluations and planning interventions.
4. Referring clients to community resources for practical and environmental needs.
5. Advocating for clients and families within community systems.
6. Participating in developing the care plan with the healthcare team.
7. Developing contracts with community agencies for patient resources.
8. Educating patients and families about community resources to promote health and safety.
9. Documenting observations and interventions accurately using electronic tablets.

Qualifications:

• Licensed by the PA Board of Social Work Examiners at LMSW or LISW level.
• Master's degree in Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education.
• Home care experience is a plus.
• Effective communication skills in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficiency with computers; experience with EMR preferred.
• Ability to travel as needed.
