BAYADA Home Health Care - Medical Social Worker (MSW) - Concord, NH

BAYADA Home Health Care is seeking an energetic and experienced Licensed Medical Social Worker (MSW) for a per diem opportunity performing home health visits at our Concord, New Hampshire office. This office services clients throughout Concord, NH on a per-visit basis.

Prior home care experience is preferred but not required. As a Medical Social Worker, you will be an integral part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to receive necessary medical care while remaining at home.

Responsibilities include:

1. Making home visits to clients in designated geographic areas.
2. Understanding social and emotional factors affecting clients' health.
3. Providing clinical social evaluations and planning appropriate interventions.
4. Referring clients and families to community resources for practical and environmental needs.
5. Advocating for clients and families navigating community systems.
6. Participating in developing the total care plan with the healthcare team.
7. Developing relationships with public and private agencies as resources.
8. Educating patients and families about community resources to promote health and independence.
9. Documenting observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Master's in Social Work (MSW) with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (degree or transcript proof).
• Home care experience is a plus.
• Effective communication skills in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficiency in computer skills; experience with EMR preferred.
• Ability to travel to cases as needed.
