Job Opportunity: Medical Social Worker, MSW

BAYADA Home Health Care is seeking an experienced Licensed Medical Social Worker (MSW) for part-time or per diem home health visits at our Harrisburg office. This role involves servicing clients across Dauphin, Cumberland, and Perry Counties.

Prior home care experience is preferred but not required. As a Medical Social Worker, you will be an essential part of a multidisciplinary team providing skilled nursing and rehabilitative care, enabling clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Understanding social and emotional factors affecting client health.
3. Providing social evaluations and planning interventions.
4. Referring clients to community resources to meet their needs.
5. Advocating for clients and families within community systems.
6. Participating in care planning with the healthcare team.
7. Developing contacts with community agencies.
8. Educating patients and families on community resources.
9. Documenting patient observations and interventions using electronic tablets.

Qualifications:

1. Licensed by the PA Board of Social Work Examiners at LMSW or LISW level.
2. Masters in Social Work with at least 2 years of community experience.
3. Accreditation by the Council on Social Work Education.
4. Home care experience is a plus.
5. Effective communication skills in English.
6. Ability to work independently and manage time.
7. Strong interpersonal skills.
8. Solid computer skills; EMR experience preferred.
9. Ability to travel as needed.
