We are a home health, hospice, and palliative care agency providing services in patient homes across 15 counties in Wisconsin. Our services include private residences, assisted living facilities, memory care units, and other home settings. Preceptor aims to deliver high-quality care through our diverse programs, with branch offices in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.

We are currently seeking a Full-Time Social Worker for our Home Care and Hospice Divisions in Racine, Milwaukee, and Kenosha County territories.

Our clients reside in assisted living facilities, private homes, or wherever they call home. We bring our services directly to them. Qualifications for the ideal Social Worker:

• Licensed to practice social work in Wisconsin
• Master's Degree preferred, from an accredited school of social work by the Council on Social Work Education
• Bachelor's Degree accepted with one year of hospice experience
• Experience in hospice and home health is preferred but not required; training will be provided for suitable candidates
• Excellent verbal and written communication skills
• Compassionate, diligent, and reliable
• Driver's license and auto insurance

Position Responsibilities:

As a Home Health and Hospice Social Worker, you will provide psychosocial services to terminally ill patients and their families.

Hospice:

• Participate in hospice admission with the nurse
• Assess psychosocial and pre-bereavement needs regularly
• Collaborate with the hospice team, patients, families, and facilities
• Develop and implement the Social Work Plan of Care
• Re-evaluate psychosocial needs periodically
• Attend weekly interdisciplinary team meetings and on-call rotations

Home Health:

• Provide resource information to families
• Assist with future planning
• Offer personalized education on health, wellness, routines, and goals
