A leading company in healthcare is seeking a Social Worker to join its Senior Care Division. The ideal candidate will be responsible for assessing and coordinating health services for members, ensuring effective discharge planning and recovery support. This is a fully remote position offering a Monday to Friday work schedule, with a focus on compassionate care.
Our Senior Care Division is excited to welcome a Social Worker to join the team! The Social Worker is a professional Population Health team member embedded within the interdisciplinary team. They are responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating services for members who face barriers with their environment, personal services, medical care, and require assistance with community resources and support services. This effective discharge planning decreases the chances of readmission to the hospital, helps in recovery, ensures medications are prescribed and given correctly, and adequately prepares member/caregivers for the recovery process. The Social Worker must obtain a CCM within two years of their hire date or already possess one. The work schedule for this fully remote position is Monday – Friday, 8:00 a.m. to 5:00 p.m. EST, occasionally extending to 9:00 a.m. to 6:00 p.m. EST.
