Preceptor Home Health & Hospice - Now Hiring: Full-Time Social Worker

Preceptor Home Health & Hospice is expanding! We provide home health, hospice, and palliative care services across 15 counties in Wisconsin, including private residences, assisted living facilities, and memory care units. Our branch offices are located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.

We are seeking a dedicated Full-Time Social Worker to join our Home Care and Hospice divisions in Racine, Milwaukee, and Kenosha County territories. Our clients reside in various settings, and our team brings essential services directly to them. Qualifications:

1. Licensed Social Worker in Wisconsin.
2. Master's Degree in Social Work preferred; accredited by the Council on Social Work Education.
3. Bachelor's Degree accepted with at least one year of hospice experience.
4. Experience in hospice and home health is preferred but not required; training will be provided for suitable candidates.
5. Excellent communication skills, both verbal and written.
6. Compassionate, hardworking, and reliable.
7. Valid driver's license and auto insurance.

Position Responsibilities:

Hospice

• Participate in hospice admission process with the hospice nurse.
• Assess psychosocial needs of patients and families, including pre-bereavement and anticipatory grief.
• Collaborate with the hospice IDG, patients, families, and facilities.
• Create and implement Social Work Plan of Care tailored to patient needs.
• Re-evaluate psychosocial needs regularly.
• Attend weekly IDG meetings and participate in on-call rotations with team members.

Home Health

• Provide resource information to families.
• Assist with future planning.
• Offer personalized education on health and wellness, home routines, and goals.
