Preceptor Home Health & Hospice - Now Hiring a Full-Time Social Worker

Preceptor Home Health & Hospice is expanding! We provide home health, hospice, and palliative care services across 15 counties in Wisconsin, including private residences, assisted living facilities, and memory care units. Our branches are located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.

We are seeking a Full-Time Social Worker for our Home Care and Hospice Divisions in Racine, Milwaukee, and Kenosha County. Our clients are served wherever they call home, and your role will be to bring compassionate support and resources during their most challenging times.

Qualifications:

• Licensed to practice social work in Wisconsin
• Master's Degree preferred; accredited by the Council on Social Work Education
• Bachelor's Degree accepted with one year of hospice experience
• Hospice and Home Health experience is a plus but not required; training will be provided
• Excellent communication skills
• Compassionate, hardworking, and dependable
• Valid driver’s license and auto insurance

Responsibilities:

Hospice:

• Participate in admission processes with the hospice team
• Assess psychosocial and pre-bereavement needs of patients and families
• Collaborate with the interdisciplinary group, patients, families, and care facilities
• Develop and implement social work care plans
• Attend weekly meetings and participate in on-call rotations

Home Health:

• Provide resource information and support to families
• Assist with future planning and health education tailored to patient needs
