Qualifications

• Master’s degree in social work (MSW) is required.
• Experience working with Medicare-aged patients, including those with serious illness or dementia.
• Ability to work independently and as part of a multidisciplinary team.
• Strong ethical standards and excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive psychosocial assessments focusing on social determinants of health, caregiver support, and gaps in care.
• Perform ongoing reassessments as patient condition evolves, identifying safety risks and caregiver strain.
• Develop patient-centered care plans with the clinical team to support risk reduction and chronic disease management.
• Ensure care plans are actionable, updated, and aligned with patient goals and community resources.
• Participate in interdisciplinary team meetings to discuss patient status and next steps.
• Provide proactive outreach to high-risk patients and engage caregivers in outreach activities.
• Educate patients, families, and caregivers to improve engagement and outcomes.
• Provide caregiver education, coaching, and emotional support, including anticipatory guidance.
