Job summary

Qualifications

• MSW from an accredited institution and licensure (LCSW) required or completed with supervision.
• Strong written and verbal communication skills and critical thinking.
• Familiarity with care management principles and health insurance concepts.

Responsibilities

• Assessment & screening for transition planning, mental health, substance use, and goals of care.
• Developing and monitoring care plans addressing social determinants of health.
• Provide brief individual, group, and family therapy when indicated.
• Identify mental, emotional, and behavioral disorders and refer to services.
• Educate and advocate for patients; train staff and support safe transitions.
• Collaborate with healthcare teams, insurers, and community providers.
• Lead quality improvement initiatives and ensure policy adherence.
• Facilitate clinical goals of care discussions with patients and families.
• Maintain ongoing documentation and coordination of care activities.
