Qualifications

• Master’s degree in social work is required; one year with individuals/families in a community setting or a Bachelor's with three years of experience.
• Experience assessing and supporting clients in healthcare or social service settings is desired.

Responsibilities

• Collaborates with medical providers, care managers, and social service agencies to coordinate short-term care and connect clients to resources.
• Develops, implements, and adjusts individualized care plans.
• Assesses high-risk clients for safety/intervention needs and provides provisional support.
• Links clients to health and social programs, including insurance and community benefits.
• Supports access to services: medical home, coverage, and barrier reduction.
• Connects clients with housing, food, clothing, prenatal education, parenting support, and mental health services.
• Explains program purposes and their impact on client wellbeing.
• Advocates for clients during calls and visits to reduce barriers and ensure needs are met.
• Delivers educational sessions on parenting, emotional regulation, sexual health, and related topics.
• Maintains accurate documentation and monthly reports.
