Job summary

Blue Cross Blue Shield of Michigan is seeking an experienced Social Worker to support members through case management in collaboration with case managers and physicians. You will help educate members about entitlement programs and develop plans of care, coordinating resources and services across the care continuum.

The role emphasizes psychosocial assessments, home health care experience, and telephonic/digital member engagement, with a focus on HIPAA compliance and ethical care delivery.

Qualifications

• Master’s Degree in Social Work or Psychology required.
• Minimum of 3 years of practice as a Licensed Master’s Social Worker required.
• Minimum of two years in home health care or medical case management preferred.
• Experience managing patients telephonically and via digital channels (mobile apps and messaging) preferred.
• Current unrestricted LMSW in Michigan is required.
• CCM certification preferred or to be obtained within 18 months of hire.
• Ability to think critically, be decisive and problem solve.
• Knowledge of HIPAA, ADA and Clinical Ethics.

Responsibilities

• Use the case management process to assess, develop, implement, monitor, and evaluate care plans to optimize health across the care continuum.
• Consult and coordinate with the case manager and physician to identify member issues and develop a plan to resolve them.
• Utilize government, community and employer resources to address member needs.
• Facilitate and coordinate Behavioral Health resources as identified.
• Manage behavioral, psychosocial needs and deliver social work interventions.
• Identify and refer members to community resources, counseling, and/or support as needed.
• Arrange resources to meet identified needs (community resources, mental health services, etc.).
• Assess progress of social work interventions via on-site visits and POD reviews.
• Assist with psychosocial needs assessment and resource identification.
• Document all activities including assessments and communications.
• Educate about entitlement programs (Medicaid, Social Security) and assist in applications.
• Assist with long-term care planning and end-of-life decisions as needed.
• Provide member support throughout hospice period.
