Blue Cross Blue Shield of Michigan seeks a Licensed Master’s Social Worker to support members by coordinating case management, applying psychosocial assessments, and connecting families with community and healthcare resources in Detroit area.

The role emphasizes collaboration with case managers and physicians, HIPAA knowledge, and a patient-centered approach to planning care across the continuum.

Formación

• Master’s Degree in Social Work or Psychology is required.
• Current unrestricted Licensed Master’s Social Worker in Michigan is required.
• Experience in home health care or medical case management preferred.
• CCM certification preferred or to be obtained within 18 months of hire.

Responsabilidades

• Use case management process to assess, develop, implement, monitor, and evaluate care plans.
• Consult with case managers and physicians to identify member issues and develop a plan to resolve them.
• Refer members to community resources and support services.
• Coordinate behavioral health resources as identified.
• Document all activities including assessments and communications.
