Job summary

UnitedHealth Group is seeking a Medical Social Worker to provide psychosocial support, assessment, and care coordination for patients and families receiving home health services. You will work with an interdisciplinary team to connect patients with community resources, offer counseling, and promote optimal outcomes in the home setting.

The role requires a Masters and/or Doctorate in social work, active LMSW/LCSW licensure, and CPR certification, with at least one year of health care social work

Qualifications

• Masters and/or Doctorate degree from a school of social work, accredited by the Council on Social Work Education (CSWE).
• Active licensure as LMSW or LCSW.
• Current CPR certification.

Responsibilities

• Assists the patient and family with health-related financial, social and emotional concerns and helps develop coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures changes in the patient’s clinical status are communicated to the health care team and care center staff in a timely manner.
• Educates the patient and family and other team members on social work interventions and the treatment plan.
• Performs care planning, transfer and discharge functions.
• Identify risks to the patient’s health and implement interventions to reduce risks, communicating with physicians and team members.
• Other duties as assigned.

Education

Master's and/or Doctorate degree in Social Work
