You will collaborate with a multidisciplinary team, utilize motivational interviewing, and ensure effective transitions from hospital to home. IL LCSW is required; Master’s in Social Work and care management certification preferred.

Qualifications

• Active Illinois LCSW license required.
• Master's Degree in Social Work from an accredited university.
• Two years' experience as a social work clinician.
• Care Management certification or plan to obtain within 2.5 years.
• Ability to coordinate complex psychosocial cases with multidisciplinary team.

Responsabilités

• Identify patients needing outreach from psychosocial and environmental perspectives.
• Assess patients' physiological and psychosocial needs; develop psychosocial care plans.
• Coordinate social services and follow-up care for complex cases.
• Collaborate with physicians, nurses, and staff to improve care transitions.
• Lead education and training on psychosocial issues within the team.
• Provide crisis intervention when needed.
• Document in EMR and MHN/EPIC with accurate terminology.
