Job summary

Silver Cross Hospital is seeking a Licensed Social Worker to join the Continuum of Care team in Illinois. The role focuses on post-acute services, evaluating patient and family needs and coordinating care to link to appropriate levels of care while addressing emotional, medical, and psychosocial requirements.

The successful candidate will provide counseling, support discharge planning, and collaborate with a multidisciplinary team to ensure culturally sensitive and compliant care delivery.

Qualifications

• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) with MSW required.
• Current CPR certification required.

Responsibilities

• Perform complex discharge planning based on Continuity of Care assessments.
• Provide counseling to patients and families regarding illness, disability, and cultural needs.
• Consult with Health Care team to set treatment goals and discharge plans.
• Coordinate with outside agencies on social and legal matters.
• Utilize insurance benefits to maximize resources and inform patients accordingly.
• Deliver CMS regulatory notices within required timeframes.
• Educate staff, patients, and families about community resources.
• Promote a safe environment of care and maintain privacy standards.

Summary: Serves as a member of a multi-disciplinary team to provide information, resources and choices for post-acute services to patients and families through assessments and care coordination efforts to effectively link them to the appropriate level of care to meet the patients emotional, medical and psychosocial needs.

Essential Duties and Responsibilities:

• Complex discharge planning based on Continuity of Care assessment and anticipated post-acute care needs in direct consultation with patient, family and multidisciplinary team which incorporates choice
• Provides counseling services to patients and their families to assist with adjustment to illness and /or disability with regards to the patients cultural, religious and ethnic background and needs
• Provides consultation to the Health Care team to establish and implement appropriate treatment goals and discharge plans with regards to the patients cultural, religious and ethnic background and needs
• Interacts with outside agencies on issues of suspected abuse/neglect/domestic violence/guardianship and other social/legal matters
• Uses knowledge of insurance benefits and coverage guidelines to maximize resources
• Delivers CMS regulatory notices within CMS established timeframes
• Educates staff, patients and families about available community resources
• Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
• Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct; maintains licensure and abides by NASW standards

Requirements: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Masters of Social Work (MSW) required. Must have current CPR.

Work Shift Details:

Afternoons, Afternoon 11a-7:30p Monday thru Friday with potential weekend/holiday as assigned

The expected pay for this position is listed below:

$32.65 - $40.81

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.
