Job summary

Silver Cross Hospital is seeking a licensed social worker to serve on a multi-disciplinary team. You will assess post-acute needs, coordinate care, and link patients and families to appropriate services while respecting cultural and personal factors.

Responsibilities include complex discharge planning, counseling, external agency liaison, CMS compliance, and staff education to ensure high-quality, privacy-conscious care. Strong licensure and CPR are required.

Qualifications

• MSW required with LSW or LCSW licensure
• Must have current CPR certification

Responsibilities

• Complex discharge planning based on Continuity of Care assessment and multidisciplinary input
• Counsel patients and families to support adjustment to illness and disability
• Consult Health Care team to set goals and discharge plans respecting cultural and personal needs
• Coordinate with external agencies on social/legal issues
• Educate patients, families and staff about community resources
• Maintain privacy and confidentiality in line with NASW standards
• Deliver CMS regulatory notices within CMS timeframes

Summary

Serves as a member of a multi-disciplinary team to provide information, resources and choices for post-acute services to patients and families through assessments and care coordination efforts to effectively link them to the appropriate level of care to meet the patients emotional, medical and psychosocial needs.

Essential Duties and Responsibilities

• Complex discharge planning based on Continuity of Care assessment and anticipated post-acute care needs in direct consultation with patient, family and multidisciplinary team which incorporates choice
• Provides counseling services to patients and their families to assist with adjustment to illness and /or disability with regards to the patients cultural, religious and ethnic background and needs
• Provides consultation to the Health Care team to establish and implement appropriate treatment goals and discharge plans with regards to the patients cultural, religious and ethnic background and needs
• Interacts with outside agencies on issues of suspected abuse/neglect/domestic violence/guardianship and other social/legal matters
• Uses knowledge of insurance benefits and coverage guidelines to maximize resources
• Delivers CMS regulatory notices within CMS established timeframes
• Educates staff, patients and families about available community resources
• Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
• Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct; maintains licensure and abides by NASW standards

Requirements

• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Masters of Social Work (MSW) required.
• Must have current CPR.

Work Shift Details

Afternoons, Afternoon 11a-7:30p Monday thru Friday with potential weekend/holiday as assigned
