Job summary

Silver Cross Hospital is seeking a part-time Medical Social Worker to join a multidisciplinary team. You will provide information, resources and care coordination to link patients and families to post-acute services, addressing emotional, medical and psychosocial needs.

Responsibilities include complex discharge planning, counseling, and collaboration with care teams while ensuring CMS timelines and privacy standards are met. Strong licensing and MSW are required, with CPR current.

Qualifications

• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Master of Social Work (MSW) are required.
• Current CPR certification is required.
• Ability to coordinate complex discharge planning and link patients to appropriate post-acute care.

Responsibilities

• Perform complex discharge planning with patient, family and care team.
• Provide counseling to patients and families to support adjustment to illness or disability.
• Consult with health care teams to set goals and discharge plans respecting cultural and religious needs.
• Interface with outside agencies on social/legal matters such as abuse or guardianship.
• Utilize insurance knowledge to maximize available resources and benefits.
• Educate staff, patients and families about community resources and services.
• Maintain privacy and confidentiality per standards; support safe care environments.

It’s the way *you*want to be treated.Summary: Serves as a member of a multi-disciplinary team to provide information, resources and choices for post-acute services to patients and families through assessments and care coordination efforts to effectively link them to the appropriate level of care to meet the patients emotional, medical and psychosocial needs. **Essential Duties and Responsibilities:*** Complex discharge planning based on Continuity of Care assessment and anticipated post-acute care needs in direct consultation with patient, family and multidisciplinary team which incorporates choice* Provides counseling services to patients and their families to assist with adjustment to illness and /or disability with regards to the patients cultural, religious and ethnic background and needs* Provides consultation to the Health Care team to establish and implement appropriate treatment goals and discharge plans with regards to the patients cultural, religious and ethnic background and needs* Interacts with outside agencies on issues of suspected abuse/neglect/domestic violence/guardianship and other social/legal matters* Uses knowledge of insurance benefits and coverage guidelines to maximize resources* Delivers CMS regulatory notices within CMS established timeframes* Educates staff, patients and families about available community resources* Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits* Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct; maintains licensure and abides by NASW standards Requirements: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Masters of Social Work (MSW) required.
