Job summary

Best Home Health Providers, Inc. is seeking a Medical Social Worker to lead and mentor our social services team in the Bay Area and beyond. The role focuses on guiding patient transitions from medical facilities to home care, ensuring high-quality service with clinical oversight.

This per diem position pays $100–$120 per visit and offers opportunities to grow into full-time roles based on performance. The candidate should hold an MSW and a California LCSW license with at least three years of

Qualifications

• MSW degree from an accredited program.
• Active California LCSW license.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and relationship-building with patients, families, staff, and community.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care and clinical documentation.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education to keep skills sharp.
• Contribute to quality improvement initiatives and identify ways to better serve patients.
• Perform other duties as required for patient services.

• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
