Job summary

Best Home Health Providers Inc. seeks a Medical Social Worker to lead our social services team in the Bay Area. You will mentor staff, oversee care planning and documentation, and partner with leadership on budgets and program standards.

This leadership role shapes patient transitions from facility to home care. You’ll use your MSW and CA LCSW credentials to guide practice, ensure regulatory compliance, and drive quality improvements across our organization.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication with patients, families, staff and community.
• Self-directed, organized and flexible.

Responsibilities

• Lead daily operations of the social services team, policies, procedures, and care coordination with clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care and ensure compliant documentation.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and patient service enhancements.
• Perform other duties as required for patient services.

As a Medical Social Worker with Best Home Health Providers, you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization.

Please note that this role is per diem with a rate of $100 - $120 per visit.

• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
