Job summary

Fives DyAG Corp. in California is seeking an Allied Health Social Worker for an acute care hospital assignment. Start date ASAP with a 13-week onsite contract. Workdays are day shift, 8 hours per day, 5 days per week.

Requires 2 years of acute medical social work experience with discharge planning and Epic charting. Travel may be required; MSW or LCSW credentials preferred. Competitive weekly stipends apply.

Qualifications

• 2 years acute medical social work experience performing both roles of social work and discharge planning.
• Epic charting system experience is required.
• MSW or LCSW certification preferred.
• Willingness to travel for assignments.

Responsibilities

• Case Management/Utilization Review.
• Care coordination and discharge planning.
• Needs assessment and ordering DME.
• Develop and implement plan of care for patients/families.
• Participate in care conferences and collaboration with multi-disciplinary teams.

RTO Restrictions: No more than 7 days
