Job summary

Community Medical Group in Miami, FL, is seeking a meticulous Medical Records II to support patient care and value-based initiatives. You will retrieve records, update the EMR, and coordinate requests while ensuring privacy and accuracy.

The role partners with the CDI team to close HCC documentation gaps and maintain auditable records in the EMR. Strong organization and bilingual skills are preferred to serve diverse patients.

Qualifications

• High school diploma required.
• Healthcare information management education preferred.
• Experience with medical records, EMR, or coding preferred.
• Experience retrieving medical records from external providers.
• Knowledge of HIPAA and patient privacy requirements.
• Strong organizational and communication skills.
• Bilingual in English and Spanish or Creole preferred.

Responsibilities

• Request and retrieve prior medical records from external providers.
• Partner with the CDI team to obtain documentation for accurate coding and care continuity.
• Document outreach and follow-up in the EMR with accurate statuses.
• Upload documents into the EMR with proper patient identification and dates of service.
• Review records for completeness, accuracy, and correct patient assignment.
• Coordinate and manage internal and external medical record requests end-to-end.
