Job summary

Qualifications

  • High school diploma required.
  • Healthcare information management education preferred.
  • Experience with medical records, EMR, or coding preferred.
  • Experience retrieving medical records from external providers.
  • Knowledge of HIPAA and patient privacy requirements.
  • Strong organizational and communication skills.
  • Bilingual in English and Spanish or Creole preferred.

Responsibilities

  • Request and retrieve prior medical records from external providers.
  • Partner with the CDI team to obtain documentation for accurate coding and care continuity.
  • Document outreach and follow-up in the EMR with accurate statuses.
  • Upload documents into the EMR with proper patient identification and dates of service.
  • Review records for completeness, accuracy, and correct patient assignment.
  • Coordinate and manage internal and external medical record requests end-to-end.

Community Medical Group is seeking a detail-oriented, organized, and dependable Medical Records II professional to join our team.

In this role, you will play an important part in supporting patient care, clinical documentation, and value-based care initiatives by ensuring medical records are accurate, complete, and available when needed. A key focus of this position will be partnering with our Clinical Documentation Improvement (CDI) team to retrieve prior medical records for new patients and help close identified Hierarchical Condition Category (HCC) documentation gaps.

You will also support the full medical records process, including reviewing and uploading documents into the Electronic Medical Record (EMR), coordinating internal and external record requests, and maintaining accurate and auditable documentation.

Key Responsibilities
  • Request and retrieve prior medical records from external healthcare providers for new patients with documentation gaps identified by the CDI team
  • Partner closely with the CDI team to obtain the documentation needed to support accurate coding, continuity of care, and value-based care initiatives
  • Document outreach, follow-up attempts, record status, and outcomes accurately within the Electronic Medical Record (EMR)
  • Prioritize medical record retrieval requests based on CDI needs, urgency, and established workflow standards
  • Process incoming medical records, faxes, scanned documents, and correspondence within established turnaround times
  • Upload documents accurately into the EMR while ensuring proper patient identification, document classification, and date of service
  • Review records for completeness, accuracy, duplicate status, and correct patient assignment before finalizing uploads
  • Coordinate and manage internal and external medical record requests from start to finish
  • Obtain records from hospitals, specialists, imaging centers, prior providers, and other healthcare facilities
  • Track and follow up on outstanding requests to ensure timely completion and resolution
  • Maintain detailed and auditable documentation of requests, follow-up activities, and final outcomes within the EMR
  • Ensure strict compliance with HIPAA regulations, patient privacy requirements, and company policies
  • Meet departmental productivity, accuracy, and workflow standards
  • Identify opportunities for process improvement and workflow optimization within the medical records function
  • Perform additional duties as assigned
Qualifications
  • High school diploma or equivalent required
  • Additional education in healthcare, medical administration, health information management, or medical coding preferred
  • Previous experience in medical records, clinical documentation, medical coding, or a healthcare office environment preferred
  • Experience requesting and retrieving medical records from hospitals, specialists, imaging centers, and other external healthcare facilities
  • Experience working with Electronic Medical Records (EMR) systems and document management processes
  • Knowledge of medical terminology and chronic conditions preferred
  • Familiarity with HCC documentation, clinical documentation improvement, or value-based care preferred
  • Knowledge of HIPAA regulations and medical record confidentiality requirements
  • Strong organizational skills with the ability to manage multiple requests and priorities effectively
  • Exceptional attention to detail and commitment to data accuracy
  • Strong written and verbal communication skills
  • Ability to work independently while following established workflows and procedures
  • Proficiency with Microsoft Office applications, email, and computer-based systems
  • Ability to maintain professionalism and confidentiality when handling sensitive patient information
  • Bilingual in English and Spanish or English and Creole preferred
