Job summary

Carespring Health Care Management is seeking a Medical Records Coordinator for a skilled nursing facility in Ohio. Responsibilities include audits, record assembly for discharged residents, and processing information requests while ensuring HIPAA compliance.

Position requires every other weekend and offers competitive wages with health insurance options, including medical and dental coverage. Experience with PointClickCare and MS Word/Excel is preferred.

Qualifications

• Maintains or is willing to obtain Medical Records Certification.
• Detail oriented with ability to meet deadlines.
• PointClickCare experience is preferred.
• Knowledge of Microsoft Word, Excel highly encouraged.

Responsibilities

• Performs routine audits of in-house charts upon admission and quarterly.
• Notifies staff on deficiencies and follows up for completeness.
• Assembles the medical records of discharged residents.
• Analyses discharged charts for completeness and follow-up with staff and physicians.
• Processes requests for medical information as directed by Administrator/Director.
• Maintains Release of Information Log for all requests and copies released.
• Completes Accounting of Disclosures form per HIPAA policies.
• Maintains confidentiality of medical records.
• Monitors physician visits and reports non-compliance monthly.
• Purges charts and maintains destruction log.
• Assigns ICD-9 CM codes for resident diagnoses in HCS system.
• Coordinates physician dictation with outside transcription company.
